New Patient Registration

The doctors welcome new patients who live within our practice area.

We are extremely happy to receive new patient registrations from patients living within the practice boundaries, which lie between the Barnstaple and Bideford Bridges, and approximately three miles inland from the coastline within this area. This includes most postcodes starting with EX31 2, EX31 3, EX32 0 & EX39 4. If you are located on the edge of our practice boundary and are unsure if your postcode is included, please check here.

We believe that health care is best delivered by having one GP responsible for an individual patient’s care and every patient registered with the practice has a usual doctor. It will not always be possible for you to see your usual doctor, but he or she is your first option when requesting a consultation.

To register, please pop into reception and you will be given a registration pack to take away with you to complete, or you may download and print a copy of our registration pack below.

Alternatively, if you wish you can fill out our online registration form on the NHS website.

Once filled out, please return all the application forms to reception along with some photographic ID (to view what we accept, please click here), and make sure that you have provided a handwritten signature where required to prove your identity.

If you need to set up your repeat prescriptions list, please provide your re-order list (the white part of the prescription slip) from your previous surgery when you register. It may be appropriate for the Pharmacist to review your medication with you and the Reception Team will contact you to arrange this.

You will have a named, accountable doctor who is responsible for coordinating your care. You can still talk to or make appointments to see any of our doctors or nurses, not just your named GP.

We are unable to register patients if no handwritten signature has been provided.

Temporary patients

If you are not registered with the surgery you will be unable to see one of our doctors unless you have a serious emergency. In most other cases, calling 111 is a better option.

If you are not registered but need treatment at the surgery you will need to complete a temporary registration form.

You can be registered as a temporary patient for up to three months. This means you can be registered with our practice and remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with our practice.
