ADHD and Autism referrals for adults and children
Referrals can be processed in two ways, both will involve your GP and both are funded by the NHS.
1. Right to Choose route via independent assessors specifically approved by NHD and Devon ICB.
View a list of current providers. If you cannot access this document, please visit the practice for a paper copy.
We would recommend you visit your chosen service providers website to check their referral criteria and also to download and complete any paperwork / questionnaires which will be required by the GP.
Follow the guidelines for your chosen assessor and contact the surgery by accuRx, we can then start the GP referral process.
2. Direct via the NHS local services (can have long waiting lists)
Children ADHD referrals:

Be sure to download the forms for both Education and Parent/Carer.
Take the Education ones to your child’s school for completion and return to the surgery together with the Parent/Carer forms all complete and then the GP can make the referrals
Children Autism referrals:
Autism referrals is via the NHS rather than Right To Choose. To self refer you can download and complete a form on the Children and Family Health Devon website. You’ll need to scroll down the forms list to ‘Neurodiversity’ where you can download the General Referral Form.
Adults autism and ADHD Service:
Please see the link below for more information about the referral process for NHS referrals, which includes up to date waiting times. Be aware we have no control over waiting times or ability to expedite these waiting times. If you would like referral direct for an NHS assessment after reviewing this information please download and complete the appropriate form then send back in either via accuRx or print it out to hand in to reception. We can then start the GP referral process, which may involve a telephone appointment if more information is needed or may be possible to send the referral directly using information from our clinical records. Please be aware that once a referral has been sent we are unable to chase this up for you, but details of how you can check waiting times can be found on the DPT website
