Proxy Access
On occasions, you may find it easier for someone else to be able to access to your information on your behalf. We call this ‘Proxy Access’.
There are a number of reasons why you might allow someone else to view your information, such as:
- You are very unwell or just need help managing your health or long term condition
- You are finding it more difficult to look after yourself due to memory issues or speech difficulties
- You have learning difficulties and would like someone else to help you understand your health
- You have a carer who can help you manage your health
- You may be planning for the future or choosing someone to hold lasting power of attorney for health and social care for you
- You are a young person and would like your parent or guardian to look after your health
- You are not comfortable with using computers, smart phones, or tablets
To request Proxy Access, you will need to fill in our Proxy Online Services Application form which you can either download below or collect from reception.

We are unable to grant proxy online services access if no ID or handwritten signatures have been provided.
Third Party Access
If you would just like someone else to be able to contact and speak to practice staff on your behalf, you will need to complete a Third Party Access form. These are also available from reception or you can download one below.
Linked profiles in your NHS account
Once Proxy Access is set up, you can access the other person’s profile in your NHS account, using the NHS App or website.
The NHS website has information about using linked profiles to access services for someone else.
