Before completing the registration form, it’s helpful to know your NHS number. If you do not know your NHS number, you can use the NHS website to find your NHS number.
** If you do not have access to the internet, or are having trouble using the online registration form, then please call in to reception (between 8:00am and 6:00pm, Monday to Friday) and someone will be able to assist you. **
If you have any repeat medication, please provide specific details of this on the online registration form; or you can provide us with your re-order list (the white part of the prescription slip) from your previous practice.
