Registering for online services

Having an online account will allow you to book appointments, request repeat prescriptions and access your medical record online.

To register for online services, please pop into reception and you will be given an online services application form to take away with you and complete, or you can download a copy below.

Once filled out, please return the application form to reception along with some photographic ID (to view what we accept, please click here), and make sure that you have provided a handwritten signature where required to prove your identity.

If you wish for your child (under the age of 16) to have online services access, the following is required:

  1. You must have an online account as patients under the age of 16 are given proxy access via their parent/guardian
  2. You will need to pop into reception and pick up an under 16s proxy online access application form to fill out

We are unable to grant online services access to patients if no ID or a handwritten signature has been provided.
