If you live outside of our practice catchment area, there is still a way that you can be registered with the practice, and this is called an ‘Out of Area Registration’.
This will allow you to attend the practice for appointments, however we will be unable to carry out any home visits. For more information about this, please read our Out of Area patient leaflet.
New Patients
If you are wishing to register with us as a new patient, please complete a registration form, and then sign one of our OOA consent forms. If the consent form hasn’t been signed, we will not be able to register you here as an ‘Out of Area’ patient.
Existing Patients
For any patients already registered with us, who have just changed address and moved out of our catchment area, please let us know by completing our change of details form; and then signing the OOA consent form. Again, if this hasn’t been done we will not be able to change your address.
